This full-time Nurse Practitioner role involves providing compassionate, high-quality palliative and hospice care to seriously ill patients in their homes, collaborating with a multidisciplinary team to address physical, emotional, and spiritual needs.

Requires a Master’s in Nursing, valid NP license, Board Certification, active NPI, and experience in pain management and hospice care.

Responsibilities include symptom management, patient education, interdisciplinary communication, discharge planning, and community education.
