We are seeking a full-time Nurse Practitioner specializing in Post-Acute Care with a focus on Cardiology or Nephrology to join a multispecialty practice.

Provide patient rounding and management at SNFs, ALFs, and LTACHs, delivering specialized care for chronic and post-acute cardiac or nephrology conditions.

Conduct assessments, develop care plans, monitor treatments, and collaborate with physicians, staff, and families to ensure high-quality, coordinated care.

Complete documentation via personal device, with flexibility to work from home and participate in an annual on-call rotation.
