Join a dedicated multispecialty practice providing exceptional post-acute care in Cardiology, Nephrology, or Pulmonary medicine as a Nurse Practitioner – Post-Acute Care (Part-Time, up to $100,000/year).

  • Perform patient rounding and management at SNFs, ALFs, and LTACHs, delivering comprehensive care for chronic and post-acute conditions.
  • Develop care plans, monitor treatments, and collaborate with physicians, staff, and families to ensure high-quality, coordinated care.
  • Complete documentation remotely using your personal device, with flexible scheduling and minimal on-call duties.
