The Nurse Practitioner role involves providing equitable, value-based primary care to adult Medicare patients in outpatient settings.

Independently conduct routine and acute visits, wellness exams, and health assessments.

Collaborate with a multidisciplinary team including physicians, medical staff, and specialists.

Lead care coordination, patient education, and population health initiatives.

Participate in outreach, phone triage, and home visits as needed.

Report to the Center Medical Director and work closely with operational leadership.

Qualifications include a Master’s or DNP degree, national certification, active licenses, and DEA authorization. Preferred experience in primary care, geriatrics, or family medicine, with knowledge of coding and risk assessments.
