Part-time/PRN opportunity for a licensed Speech Language Pathologist (SLP) to assess, diagnose, treat, and prevent communication and swallowing disorders in patients’ homes.

Responsibilities include:

Conducting initial assessments

Creating individualized treatment plans

Collaborating with the care team to ensure continuous patient care

Documenting notes in the health information system

Must have a current SLP license and a valid driver’s license. The role involves working with patients recovering from stroke, TBI, illness, surgery, injury, or dementia.
