The Speech Language Pathologist in Home Health provides assessment and therapy services to residents in a skilled nursing setting.

  • Conducts screenings, evaluates patient needs, and develops personalized treatment plans aligned with physician orders.
  • Monitors progress, adjusts interventions, and supervises therapy assistants to ensure effective care.
  • Creates programs to maintain and improve residents’ functioning within their environment.
  • Reviews service quality and treatment effectiveness regularly.
