Seeking a Full-Time Nurse Practitioner or Physician Assistant to provide primary care in skilled nursing and assisted living facilities in Falmouth, MA, and surrounding areas. Candidates must have acute care experience.

Perform primary care visits, medication reconciliation, and annual wellness exams five weekdays.

Examine patients, review medical records, order and interpret diagnostic tests, diagnose conditions, and prescribe medications.

Educate patients and families on health conditions, prevention, and treatment plans.

Maintain accurate records, oversee overall patient health, and promote good health practices.
