Seeking a Full-Time Nurse Practitioner or Physician Assistant to conduct primary care visits in Assisted Living Facilities in Hyannis, MA, and nearby areas.

Responsibilities include:

Performing primary care visits, medication reconciliations, and annual wellness exams 5 days a week

Examining patients and reviewing medical records

Ordering and analyzing diagnostic tests

Diagnosing and treating health conditions and illnesses

Prescribing medications and maintaining accurate records

Educating patients and families on health issues and prevention

Overseeing overall patient health and promoting wellness

Engaging in research and professional development
