Join a dedicated PACE team in Springfield, MA as a full-time Master of Social Work (MSW) to support aging adults in maintaining independence and dignity.

  • Conduct psychosocial assessments and create personalized care plans.
  • Provide counseling, advocacy, and support to participants and families.
  • Collaborate with an interdisciplinary team for holistic care coordination.
  • Assist with crisis intervention, discharge planning, and resource referrals.
  • Ensure compliance with regulations and documentation standards.

    • Qualifications include an MSW, with preferred licensure (LCSW/LICSW) and experience in geriatric or long-term care settings. Bilingual skills are a plus.
