Seeking a Speech Language Pathologist for home care services, providing personalized evaluation and treatment to help patients improve communication, cognition, and swallowing in their homes.

Conduct assessments, develop evidence-based care plans, and adjust strategies based on patient progress.

Document all interventions and collaborate with a multidisciplinary team to ensure coordinated care.

Educate patients and caregivers on communication and swallowing safety, fostering strong relationships.

Maintain licensure and stay current with clinical best practices; mentor peers as needed.

Qualifications include a Master’s degree, CCC from ASHA or CFY, NY State licensure, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
